Bootstrap spec.md in the current project.

Trigger: "Initialize spec.md" or /spec.md.init

Creates .spec.md/ scaffolding and generates AGENTS.md from a context discovery scan of the repository (stack, lint, tests, CI).

Present AGENTS.md for review before creating tasks.
